Staff
Olive branch nurses and caregivers are specially trained to deal with the emotions and the physical needs of our patients and families during this transitional time. They will gently care for your loved one with the medical expertise needed to professionally manage their symptoms and holistic care.
Olive Branch provides a nurse 24 hours, 7 days a week to assess and facilitate hospice or palliative care providing the appropriate and best level of care. Hospice is a support system for the family and we will be there every step helping transition the patient to the next level of care when needed.
Our Care Options
Advanced Illness Support
Home visits happen as needed by an expert Nurse Practitioner: Beginning as early as diagnosis or at any stage of serious illness. Help in understanding illness and coordinating care. Intermittent visits as needed. Identify priorities and goals for care. Identify when patient qualifies for higher level of support
Palliative Home Health Care
Team support is provided for people with pain, stress or other symptoms due to serious illness: Team members make home visits as ordered by your doctor. Nurses on call 24/7 for questions or a crisis. Help to reduce ER visits and hospital stays. Education for both patient and family. Keep receiving treatment.
Hospice at Home or in a Care Facility
When the focus shifts to comfort instead of treatment, help is there for the whole family: Team members visit regularly to provide care and support. Nurses on call 24/7 for questions or a crisis. Care tailored to each situation. Expert pain and symptom management. Emotional and spiritual support.
Where to Receive Care
Live where you want to live and we will help you to receive the care that helps you to stay there longer. You can choose where to receive Hospice or Palliative Care.
Your choice may be staying in your own home or with a friend or family. You may be in an independent or assisted living facility. We will also provide care to you in a long-term care facility.
Palliative Care and Hospice Care are available where you live. We’ll help you stay out of the hospital and avoid unnecessary visits to the ER by bring the right care at the right time. Your team will work with you to set goals, to honor your wishes and to keep you independent as long as possible.
